Impact of Your Role:
As a Licensed Practical Nurse (LPN) in Hospice, your daily contributions significantly enhance the quality of life for our patients. You will provide essential one-on-one care, utilizing your clinical expertise to support patients in their preferred environment during their end-of-life journey.
Responsibilities:
In this role, you will deliver skilled care as directed by the attending physician and in accordance with the care plans established by the RN Case Manager (RNCM). You will operate independently, requiring effective time management skills. Your responsibilities will include:
- Care Planning: Implement care plans by adhering to orders and notifying the RNCM of any patient condition changes.
- Patient Monitoring: Observe and assess patient conditions, ensuring medication reconciliation as prescribed.
- Family Support: Educate and advocate for patients and their families regarding disease progression and care plans.
- Documentation: Complete all clinical documentation in line with agency protocols and Medicare guidelines.
- Compliance: Understand and adhere to agency policies and communicate any changes in availability.
- Training: Participate in mandatory training and agency meetings.

Qualifications:
To be considered for this position, candidates must possess:
- A current Licensed Practical Nurse (LPN) license in the state of employment.
- A minimum of one year of experience as an LPN in an acute care setting is preferred.
- Experience in hospice care is desirable.
- A valid driver's license and auto insurance.
- The ability to meet all physical demands of the role.
