Impact of Your Role:
As a Licensed Practical Nurse (LPN) in Hospice, your daily contributions significantly enhance the lives of our patients. You will provide them with the invaluable opportunity to spend quality time with their loved ones in a comfortable environment.

In this role, you will deliver personalized patient care and apply your clinical expertise in end-of-life care as directed by the attending physician. You will collaborate with an interdisciplinary team dedicated to delivering compassionate, high-quality care.

Work Responsibilities:
You will provide thorough skilled care as prescribed by the attending physician, adhering to the order sets established by the RN Case Manager (RNCM). Your ability to work independently and manage your time effectively is essential.

Key Responsibilities:

  • Care Planning: Deliver care in accordance with the order sets and objectives established by the RNCM, promptly notifying them of any changes in patient status.
  • Patient Safety: Monitor and assess patient conditions, ensuring medication prescribed by the physician is accurately reconciled.
  • Family Support: Educate and advocate for patients and their families regarding disease progression, involving them in the care plan development.
  • Documentation: Complete all clinical documentation in line with agency protocols and Medicare/Federal guidelines.
  • Compliance: Adhere to agency policies, procedures, rules, and regulations, communicating any changes in schedule or availability to supervisors.
  • Training: Participate in mandatory agency meetings and in-service training sessions.

Essential Skills:

  • Empathetic communicator with a positive demeanor.
  • Patience when interacting with patients, families, physicians, and colleagues.
  • Strong attention to detail, observant, and capable of following instructions accurately.

Qualifications:

  • Current Licensed Practical Nurse (LPN) license in the state of employment.
  • Minimum of one year of experience as an LPN in an acute care environment is highly recommended.
  • Experience in hospice care is preferred.
  • Valid driver's license and auto insurance required.
  • Ability to meet all physical demands of the role.
