About the Role

We are seeking a skilled Physical Therapist Assistant to join our team at CenterWell Home Health. As a Physical Therapist Assistant, you will play a vital role in providing high-quality patient care and contributing to the success of our home health care team.

Key Responsibilities
  • Administer physical therapy treatment plans developed by Physical Therapists
  • Provide non-complex active and passive manual therapeutic exercises, modalities, and other interventions to patients
  • Accurately document patient records and submit clinical paperwork in a timely manner
  • Attend educational meetings and contribute to department in-services
  • Notify Physical Therapists or Executive Director of equipment failures, supply shortages, staff changes, and other departmental issues
  • Schedule clients and perform various office tasks
  • Apply concepts of infection control and universal precautions in patient care activities
  • Accept clinical assignments consistent with education and competence to care for patients
  • Promote compliance with fiscal intermediaries and third-party payors through education, coaching, and assistance
Requirements
  • Associate degree in Physical Therapy from an American Physical Therapy Association
  • Current state license to practice Physical Therapy
  • Valid driver's license, auto insurance, and reliable transportation
  • Proof of current CPR certification
  • Minimum two years experience as a Physical Therapy Assistant
  • Comprehensive knowledge of geriatric population changes
  • Self-directed, enthusiastic, and accepts constructive feedback
  • Good verbal and written communication and organization skills
  • Professional image and attitude for patients, families, clients, coworkers, and others
  • Maintains clinical competency in Physical Therapy practice and theory
  • Ability to work a flexible schedule and travel locally
  • Able to maintain confidentiality
  • Maintains current licensure certifications and meets mandatory continuing education requirements
  • Fluent English language skills
  • Good and regular attendance
