Hospice RN Admissions Job Description

At Custom Care Hospice, we are dedicated to providing exceptional end-of-life care to our patients. As an Admission Registered Nurse, you will play a pivotal role in educating patients, families, and caregivers about hospice philosophy and goals.

Your excellent communication and active listening skills will guide patients and families through the admission process with compassion and empathy.

Duties and Responsibilities:
  • Conduct comprehensive assessments of patients to determine eligibility for hospice care
  • Interview and educate patients' family and/or caregivers about hospice philosophy and goals
  • Review medical records and communicate with the Hospice Medical Director Clinical Leadership Team
  • Develop an initial plan of care based on hospice diagnosis
  • Complete all admission documentation in a timely manner
  • Order medication and coordinate with the pharmacy
  • Perform other duties as assigned
Qualifications:
  • Registered nurse with current unencumbered licensure to practice professional nursing in Texas
  • Prior Hospice nursing experience preferred, but not required
  • Excellent observation, problem-solving, verbal and written communication skills
  • Ability to organize and prioritize workload independently
  • Current CPR certification
  • Must be a licensed driver with a reliable and insured automobile
  • Self-directed and able to work with minimal supervision
