Clinical Assessment Advisor

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Clinical Assessment Advisor in Home Health. As a key member of our care team, you will play a vital role in ensuring the highest quality of care for our patients.
Key Responsibilities- Conduct comprehensive assessments of patients' needs and develop personalized care plans to achieve optimal clinical outcomes.
- Collaborate with healthcare professionals, including physicians and other licensed clinicians, to ensure seamless care coordination and effective communication.
- Oversee the development and implementation of care plans, ensuring they are aligned with patients' goals and needs.
- Monitor patients' progress and make adjustments to care plans as necessary to ensure optimal outcomes.
- Participate in quality improvement initiatives to enhance the overall quality of care and patient satisfaction.
- Current licensure as a Registered Nurse in the state of practice.
- At least 2 years of experience in a clinical management role, preferably in home health care.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Knowledge of state and federal regulations governing home health care, including OASIS and PDGM.
- Ability to work independently and as part of a team, with a strong focus on patient-centered care.
