Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Clinical Assessment Advisor in Home Health. As a key member of our care team, you will be responsible for conducting comprehensive assessments of patients in their homes, developing and implementing individualized care plans, and coordinating with healthcare professionals to ensure optimal patient outcomes.

Key Responsibilities:
  • Conduct thorough assessments of patients in their homes to identify their unique needs and develop personalized care plans.
  • Collaborate with healthcare professionals, including physicians, therapists, and social workers, to ensure seamless care coordination and optimal patient outcomes.
  • Develop and implement individualized care plans that address the physical, emotional, and social needs of patients.
  • Provide education and support to patients and their families to promote health and wellness.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
Requirements:
  • Current licensure as a Registered Nurse in the state of practice.
  • At least 2 years of experience in home health care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of home health care regulations and standards.
