Job Summary:

Acentra Health is seeking a skilled Clinical Assessor to join our team in North Carolina. As a Clinical Assessor, you will conduct assessments to determine the eligibility of individuals for Medicaid-funded personal care services and home and community-based services. You will work independently, traveling to assessment locations in your region, and will be responsible for ensuring that services are provided on a needs basis, in quantities appropriate to the beneficiary's unmet needs.

Key Responsibilities:
  • Conduct assessments to determine eligibility for Medicaid-funded personal care services and home and community-based services.
  • Ensure that services are provided on a needs basis, in quantities appropriate to the beneficiary's unmet needs.
  • Develop level of care recommendations based on clinical evaluations.
  • Participate in training of PCS stakeholders as needed.
  • Consult with the beneficiary's selected case management entity to generate an approvable service plan.
  • Ensure that the randomly selected Service Plan completed by the beneficiary's assigned case management entity is appropriate to the beneficiary's unmet needs.
Requirements:
  • Registered Nurse or LCSW license by the state of North Carolina.
  • 2+ years of experience in home care setting preferred.
  • Experience with community-based individuals needing personal assistance with ADL and IADL tasks is highly preferred.
  • Knowledge of North Carolina Medicaid Clinical Coverage Policy (Clinical Policy) 3L and PCS Program Provider Manual.
  • Knowledge of standards of practice related to Medicaid-funded Personal Care Services, home and community-based services (HCBS) programs, and EPSDT.
