The LPN Care Coordination Specialist plays a crucial role in ensuring high-quality, patient-centered care.

In this position, the LPN engages in a variety of advanced functions within a collaborative care team, addressing the needs of both the healthcare professionals and the patients/support network.

The Care Coordination Specialist, LPN is responsible for overseeing the management of care through a structured care delivery system.

Key Responsibilities:
This role necessitates exceptional communication skills and the ability to build relationships with referral sources.

Promote safe and effective patient environments and the proper use of feeding equipment.

Exhibit the capability to deliver care or services while adapting approaches to accommodate the developmental levels and cultural differences of the populations served.

Respond promptly to inquiries, concerns, or emergencies from staff, patients, and referral sources.

Facilitate communication with referral sources to address questions and resolve issues. Manage and triage incoming calls, directing all clinical assessments and interventions to the Clinical Team.


Ensure that patients receive exceptional service, education, and solutions regarding enteral nutrition by promoting supplies and providing essential education to achieve and maintain optimal clinical outcomes.

Contact patients and caregivers to assess enteral formula and supply needs while maintaining a computerized schedule for reorder dates. Coordinate the delivery of enteral formula and supplies to patients.

Provide education to patients and the community about all aspects of home infusion and enteral nutrition services. Oversee the initial delivery of enteral formula and supplies to patients.

Schedule nursing visits for FHI nursing staff, adhering to established productivity standards, clinical expertise, and staffing availability. The Coordinator is responsible for managing and maintaining scheduling for patients and staff. Address customer inquiries regarding the status of their visits, orders, or supplies while delivering outstanding customer service.

Demonstrate excellent communication and educational skills, along with a strong commitment to both external and internal customer service.

Possess the ability to thrive in a fast-paced, high-pressure environment.

Exhibit strong organizational and prioritization skills.

Work effectively with all departments and services.

Demonstrate computer literacy.

Have knowledge of infusion and enteral therapy access devices and administration systems.


Act as the primary point of contact for enteral patients and their families, addressing inquiries and concerns with empathy and professionalism.

Provide ongoing support and follow-up to ensure patient satisfaction and adherence to enteral nutrition therapy.

Manage inventory and ensure timely ordering and delivery of enteral nutrition supplies and equipment.

Maintain accurate records of supplies, ensuring proper documentation and compliance with regulatory standards.

Coordinate with suppliers and vendors to resolve any issues related to supply availability or quality.

Qualifications:
Required

Education

Completion of an accredited LPN program.

Experience

A minimum of two years of recent experience in home care, long-term care, clinic, urgent care, or med-surg/acute care settings.

License/Certification/Registration

Current LPN license in the State of Minnesota.

Basic Life Support (BLS)

Preferred

Education

Bachelor's Degree

Experience

A minimum of two years of recent experience in home infusion therapy, including direct patient care.

Previous customer service experience is preferred.
