Adoration Home Health and Hospice is seeking a compassionate and skilled Licensed Practical Nurse to join our team. As a LPN, you will provide high-quality care to patients in the comfort of their own homes.

**Job Summary:**

We are looking for a dedicated and caring LPN to work under the supervision of a Registered Nurse. Your duties will include providing routine care, observing client health status, and assisting the RN and physician in treatments. You will also communicate with clients and their families/guardians, reporting changes in condition and/or additional client needs to your supervisor.

**Responsibilities:**

* Provide 1:1 care for clients following their plan of care, maintaining their dignity and independence
* Monitor client vital statistics and report abnormal findings to the Case Manager and Physician
* Administer medications, feedings, oxygen, and ostomy care as needed, assessing results
* Complete routines established by other care professionals, such as PT, OT, and Speech Therapists
* Ensure infection control policies are maintained, reporting possible communicable diseases or infections
* Accurately document nursing actions, communication with family, case manager, and physicians, and progress and outcomes for established goals
* Coordinate or order supplies, medications, and feedings as needed
* Ensure all patient needs are met in accordance with physician instructions
* Monitor incidents involving individuals, completing incident reports timely and accurately
* Ensure all medical equipment is maintained and report faulty equipment to DME for repair

**Qualifications:**

* LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
* Valid driver's license, acceptable driving record, and proof of car insurance
* CPR professional certification
* Experience in administering medications
* 1+ years of nursing experience preferred
* Home health care experience preferred
* Able to lift and carry items weighing up to 20 pounds
* Able to bend, stoop, push, pull, sit, and walk for periods of time
* Must be able to communicate both verbally and in writing
