Job Details

Cheboygan Home Health & Hospice is seeking a skilled Occupational Therapist to join our team. As a Home Health Occupational Therapist, you will provide high-quality care to patients in their homes, promoting independence and physical and mental functioning.

Key Responsibilities:
  • Conduct initial and ongoing comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Evaluate patients' significant others and home situations to determine necessary instruction and family assistance.
  • Assure that physicians' orders are appropriate and discuss necessary changes.
  • Develop and implement interdisciplinary patient care plans, including Occupational Therapy in consultation with the physician.
  • Guide and instruct patients in prescribed therapeutic self-care and creative activities to improve independence and physical and mental functioning.
  • Teach, supervise, and counsel patients and their families in the total Occupational Therapy program and other related problems.
  • Establish goals based on patients' current level of functioning and potential for improvement, and perform re-evaluations as indicated.
  • Make arrangements for outpatient services when procedures are required that cannot be given in the home.
  • Evaluate the effectiveness of Occupational Therapy intervention and update patient care plans as needed.
Requirements:
  • Graduate of an Occupational Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Occupational Therapy Association.
  • Preferably a member of the American Occupational Therapy Association.
  • At least one year of Home Health Care experience.
  • Comprehensive knowledge of general therapy theory and practice, and ability to work independently.
  • Licensed or registered in the State of Michigan.
  • Must pass a criminal background check and MVR check.
  • Completed health statement acknowledging ability to perform the duties of the position.
  • Valid state driver's license and proof of current automobile insurance.
  • TB testing per agency policy.
