DescriptionThe Care Team Home Health & Hospice is seeking a skilled Occupational Therapist Assistant to provide high-quality care to patients in Cheboygan, Emmet, and Antrim counties, MI, and surrounding areas.
Key Responsibilities:- Provides skilled occupational therapy services on an intermittent basis to patients in their homes under the supervision of the Occupational Therapist.
- Assists in the evaluation and development of the rehabilitative plan of care and periodic re-evaluation as required.
- Participates in the preparation of clinical and daily progress notes.
- Prepares periodic summary reports.
- Instructs patients and family members in exercise programs and self-care techniques.
- Observes and records activities and findings in the clinical record.
- Reports physical findings and changes in the patient's condition to the Occupational Therapist and physician.
- Consults with the Occupational Therapist regarding the outcome of home visits.
- Assists in the evaluation and development of the rehabilitative plan of care and periodic re-evaluation as indicated.
- Collaborates with the patient and family to learn appropriate self-care techniques.
- Submits documentation on time per agency policy.
- Must have at least one year of Home Health Care experience.
- Comprehensive knowledge of general therapy theory and practice.
- Must be computer literate and able to maintain simple records in English.
- Must be able to effectively communicate in English.
- Must be able to drive and access patients' homes.
- Must be a graduate of an approved school giving a course of not less than two years for Occupational Therapy Assistants.
- Licensed or registered in the State of Michigan.
- Must pass a criminal background check and MVR check.
- Completed health statements acknowledging the ability to perform the duties of the position.
- Valid state driver's license.
- Proof of current automobile insurance.
- Current CPR card.
