Role Overview

The Hospital Case Manager RN Trainee is a vital member of our healthcare team, responsible for coordinating care across the continuum. This role involves working closely with physicians, peers, and community colleagues to identify opportunities for improving patient care and services, as well as population health. Our ideal candidate will possess excellent clinical skills, leadership abilities, and a strong understanding of healthcare financial and reimbursement knowledge.

Key Responsibilities

  • Collaborate with healthcare professionals to develop and implement patient care plans
  • Conduct thorough assessments of patient needs and develop strategies for improving care
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Stay up-to-date with the latest healthcare trends, regulations, and best practices

Requirements

  • Bachelor's degree in Nursing or equivalent experience
  • Current Registered Nurse license in Washington State or Multistate License endorsement
  • Five years of recent clinical hospital/ambulatory experience
  • Healthcare financial and reimbursement knowledge preferred
