Care Manager RN

We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team at Providence Medford Medical Center. As a key member of our care management team, you will be responsible for developing and managing patient care outcomes for an entire patient case load.
Key Responsibilities- Perform comprehensive assessments of patients to identify their needs and develop individualized care plans.
- Collaborate with healthcare providers, patients, and families to ensure seamless transitions of care and optimal patient outcomes.
- Utilize clinical expertise to analyze patient data and make informed decisions about patient care.
- Develop and implement effective discharge plans to ensure patients receive the necessary support and resources after hospitalization.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Current Oregon Registered Nurse License.
- Three years of acute care hospital experience.
- Preferred qualifications include experience in home health, mental health, substance use, hospice, and/or utilization review, as well as progressive nursing leadership experience.
