Job Summary

We are seeking a highly skilled and experienced RN Transplant Coordinator to join our team at Dignity Health. The successful candidate will be responsible for coordinating the clinical process from patient referral to treatment and follow-up, serving as a resource to staff and the multi-disciplinary team caring for program patients.

Key Responsibilities
  • Coordinate the clinical process from patient referral to treatment and follow-up
  • Serve as a resource to staff and the multi-disciplinary team caring for program patients
  • Demonstrate knowledge and skills in the care of patients from adolescent to adult
  • Play a role in the holistic evaluation of patient(s)
  • Maintain program compliance and QAPI
  • Comply with regulatory, governing, and other external agencies requirements
  • Assist in developing policies and procedures, order sets, and pathways to improve overall quality of care and program development
Requirements
  • Nursing degree
  • Three years experience with patients in a related setting and/or three years experience in an intensive care, acute care, telemetry, or medical/surgical setting
  • Working knowledge of regulatory compliance and survey processes
  • Joint Commission and CMS standards
  • Working knowledge of QAPI process
  • Strong verbal, written communication skills
  • Proficient computer skills
  • Performance improvement tools and methodologies such as PDCA, Six Sigma, Lean, etc.
  • Current Arizona RN license (or compact state)
Preferred Qualifications
  • BSN
  • Critical care/cardiothoracic experience
  • Program Management/Development
