- Develop and implement individualized care plans, incorporating evidence-based practices and patient-centered care principles.
- Monitor and evaluate patient outcomes, making adjustments to care plans as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Current licensure as a Registered Nurse in the Commonwealth of Massachusetts.
- 2+ years of experience in behavioral health nursing, with a focus on patient-centered care and evidence-based practices.
- Graduate of an accredited school of nursing, with a Bachelor's degree preferred.
- Valid/Current Basic Life Support (BLS) certification.
