About the Role

We are seeking a skilled Home Health Aide to join our team at East Boston Neighborhood Health Center. As a Home Health Aide, you will provide in-home assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) to our patients. This is a part-time, benefits-eligible position that requires a minimum of one year of experience working with a frail or elderly population.

Responsibilities
  • Monitor patient ADL/IADL care plans and follow the plan of care as stated.
  • Document care provided on participant flow sheets.
  • Check the health aide's voicemail at the beginning of each shift and every two hours until the end of the shift.
  • Report any abnormal findings, alterations in participant status, and/or care refusal to the team in a timely fashion.
  • Assist participants with ADLs per assignment, as defined in ADL/IADL care plan and as needed.
  • Accompany participants to appointments as assigned.
  • Carry out simple treatments under the direction of nursing staff.
Requirements
  • Current certification as an H.H.A. /C.N.A. from an approved program.
  • Must demonstrate competencies before providing participant care.
  • A minimum of one year of experience working with a frail or elderly population.
  • Prior experience as a health aide, personal care assistant, or similar role.
  • Vehicle to travel from home to home preferred.
  • Excellent customer service and communication skills.
  • Physical capability to provide ADL and mobility assistance to all PACE participants.
