Job Summary

Eden Health is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our hospice care team. As a Hospice LPN, you will provide high-quality, patient-centered care to individuals in need of end-of-life support.

Key Responsibilities
  • Provide skilled nursing care to patients in accordance with physician orders and the plan of care.
  • Collaborate with the interdisciplinary team to coordinate hospice services and ensure seamless care delivery.
  • Assess patient needs and provide education to patients and their families/caregivers on disease management and care planning.
  • Participate in quality management initiatives to ensure high-quality care and patient satisfaction.
  • Document patient care and progress in the Electronic Health Record (EHR) in a timely and accurate manner.
Requirements
  • Graduate of an accredited nursing program.
  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in the state of practice.
  • Experience as an LPN/LVN, preferably in an acute care setting or home care environment.
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance and a clean driving record.
  • Current CPR certification.
  • Able to pass a criminal background check and national sex offender clearance.
