Texas, United States Astella Home Health and Hospice Full time

***This position requires overnight travel across Texas, with all travel expenses covered.***

The role involves delivering and overseeing home healthcare services, adhering to agency protocols, and employing the nursing process effectively.

KEY RESPONSIBILITIES:

1. Deliver specialized nursing services in line with the treatment plan authorized by the physician, initiating the initial evaluation visit for clients referred by the care manager. Exhibit proficiency in essential nursing skills such as venipunctures, infusion therapy, wound management, and aseptic techniques.

2. Assess and continuously evaluate the nursing requirements of clients; develop, implement, and adjust the care plan as necessary. Monitor ongoing care needs and address any issues within the care plan, documenting the rationale for any omissions.

3. Initiate diagnostic, preventive, and rehabilitative nursing actions as appropriate to ensure client safety. Make referrals to other healthcare professionals based on client needs or document reasons for not doing so.

4. Monitor and report any changes in client condition to the physician or relevant health professionals as required.

5. Educate and guide clients and their families on home nursing practices and care needs relevant to their health status, utilizing agency educational resources.

6. Coordinate comprehensive care plans and ensure continuity of care by collaborating with other healthcare professionals involved with the same clients. Participate in client care meetings and initiate discussions for complex or multidisciplinary cases as needed.

7. Provide supervision and training to LPNs/LVNs and home health aides. Regularly assess the need for unskilled care and update the care plan accordingly, communicating any changes to the aides and scheduling personnel as necessary.

8. Create, maintain, and update individualized client care records with precision and in a timely manner, ensuring compliance with agency policies. Submit documentation promptly following client visits.

9. Adhere to all agency policies and procedures related to service delivery, documentation, and care coordination. Demonstrate compliance during supervisory visits and articulate knowledge of agency protocols, including client education, infection control, and management of hazardous materials.

10. Engage in the agency's quality improvement initiatives.

QUALIFICATIONS:

1. Nursing degree (A.D.N. or B.S.N) from an accredited institution.

2. Valid nursing license in the state.

3. At least one year of nursing experience is required.

4. Knowledge and Skills:

a. Proven ability to provide care and communicate effectively with the primary population served by the agency, and to a lesser extent, secondary populations.

b. Understanding of growth and development principles across the lifespan.

c. Capability to assess client data and interpret necessary information to identify age-specific needs.

TRAVEL REQUIREMENTS:

All travel expenses will be covered.

Daily home visits and weekly office meetings are expected.

Reliable transportation and agency-required liability insurance are mandatory.

A valid Texas Driver's License is required.
