Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient and family needs, and develop individualized plans to address these needs.
  • Collaborate with the healthcare team to ensure timely and efficient discharge planning, and provide linkage to community resources and UH network services.
  • Provide guidance and consultation to peers and team members on routine psychosocial issues and discharge planning.
  • Supervise social work interns, if eligible.
Preferred Qualifications
Education
  • Master's degree from an accredited school of social work.
Work Experience
  • Previous experience in hospital or health agency settings, preferably with substance use disorders and mothers.
Licenses and Certifications
  • Licensed Social Worker (LSW) required.
  • Licensed Independent Social Worker (LISW) within one year preferred.
Physical Demands
  • Standing, walking, and lifting frequently.
  • Pushing, pulling, and grasping frequently.
  • Talking and hearing constantly.
