We are seeking a highly skilled Registered Nurse Case Manager to join our team at Inova. As a key member of our healthcare team, you will be responsible for providing discharge planning and continuity of care for assigned patients in acute and post-acute settings.
Key Responsibilities- Discharge Planning and Coordination: Develop and implement effective discharge plans to ensure a smooth transition of patients from hospitals to the discharge setting and ongoing care in the community.
- Utilization Management: Use utilization management techniques to determine the medical necessity, appropriateness, and efficiency of the use of healthcare services, procedures, and facilities.
- Clinical Performance Improvement: Participate in clinical performance improvement activities to drive practice changes and positively impact outcomes.
- Communication and Collaboration: Collaborate with internal and external case managers, patients, families, and interdisciplinary healthcare team members to ensure effective patient management and coordination of services.
- Education and Advocacy: Provide educational resources and referrals to patients and families to address identified needs, and act as an advocate for patients to resolve barriers to care progression.
- Education: Bachelor's Degree in Nursing (BSN) or Associate's Degree in Nursing (ADN) with a commitment to obtain BSN within two years of hire date.
- Licensure: Licensed or eligible for licensure in the Commonwealth of Virginia as a Registered Nurse (RN).
- Experience: One year of Case Management and/or Clinical Care experience.
- Certifications: Basic Life Support (BLS) through the American Heart Association prior to start date.
