Position Overview:
We are seeking a dedicated Ambulatory Care Nurse who will play a vital role in the coordination and delivery of nursing services. The successful candidate will be instrumental in implementing clinical policies and ensuring the highest standards of patient care. This role involves engaging in quality improvement initiatives, systematically gathering patient health data, and collaborating with healthcare providers to formulate individualized care plans aimed at achieving optimal health outcomes.
Key Responsibilities:
The Ambulatory Care Nurse will be responsible for:
- Conducting thorough physical assessments and diagnostic evaluations.
- Gathering comprehensive patient health histories.
- Documenting patient information accurately in electronic health records.
- Providing education and support to patients and their families regarding treatment options and results.
- Administering medications, including vaccinations and IV therapies, and performing patient triage for clinical inquiries.
- Monitoring and evaluating patient responses to treatments and interventions.
- Interpreting patient data to make informed clinical decisions.
- Collaborating with healthcare teams to determine the most effective treatment strategies.
- Supervising and guiding support staff to enhance patient care delivery.
- Performing additional duties as required to support departmental objectives.
Qualifications:
Applicants should possess a Bachelor’s degree in Nursing; candidates with a Diploma or Associate’s Degree must have at least one year of relevant experience and be actively pursuing a Bachelor’s degree. A current Registered Professional Nurse license in New York is required, along with Basic Life Support Certification from the American Heart Association. Proficiency in English, both written and spoken, is essential.
Preferred Qualifications:
Certification in Advanced Cardiac Life Support (ACLS) and membership in relevant professional organizations are advantageous.
Effective communication skills are crucial for success in this role, enabling collaboration across all levels of the organization.
