Position Overview

Under the guidance of the Program Director, the Housing Case Manager plays a vital role in assisting adults facing serious mental health challenges and/or substance use issues within our supportive housing initiative.

Key Responsibilities:

  • Facilitate the achievement of client rehabilitation objectives by applying psychosocial rehabilitation strategies.
  • Establish and nurture a respectful, compassionate, and empowering rapport with clients.
  • Provide instruction in independent living skills, including but not limited to cleaning, budgeting, and conflict resolution among housemates.
  • Manage the 30-day homeless prevention initiative, collaborating with crisis residential services to evaluate client suitability and coordinate placements.
  • Maintain an assigned caseload of clients.
  • Ensure a minimum of 65% billable service time relative to total work hours.
  • Deliver supportive counseling tailored to the client's immediate needs and circumstances, utilizing Cognitive/Behavioral interventions to help clients recognize their strengths and pursue their goals.
  • Foster relationships with property managers to enhance tenant-landlord interactions.
  • Utilize personal or agency vehicles to transport clients to treatment locations, ensuring accurate documentation of mileage as per agency protocols.
  • Assist clients in submitting maintenance requests.
  • Educate clients regarding their rights and responsibilities as tenants.
  • Conduct property inspections for agency client tenants to assess living conditions and report findings to the housing manager.
  • Oversee vendor repairs and installations.
  • Support clients during lease reviews and move-in inspections.
  • Assist with client relocations and the preparation of units for new tenants.
  • Engage with homeless service providers to facilitate client placements in prevention programs.
  • Build and maintain connections with local board and care facilities to support client placements.
  • Help clients secure funds for security deposits from local charitable organizations when appropriate.
  • Document all objectives and interventions promptly in compliance with agency, quality assurance, and HUD standards.
  • Organize and file documents efficiently to maintain an orderly office environment.
  • Collaborate effectively with supervisors, case management team members, and other service providers, sharing pertinent information regarding interventions.
  • Participate in staff meetings and other assigned gatherings.
  • Engage in regular supervision sessions and complete required training in a timely manner.
  • Contribute to a safe, healthy, and clean workplace in alignment with applicable laws and the agency's Health & Safety Program.
  • Submit accurate timesheets and absence reports to program management punctually.
  • Promote the philosophy and practice of social rehabilitation within the agency and to the public.
  • Encourage a cooperative and respectful work environment that aligns with the organization's mission and values.
  • Perform additional duties and special projects as assigned.

Qualifications:

  • Demonstrated passion for the mission of Caminar Inc.
  • Preferred candidates are those registered with the California BBS; opportunities for licensed supervision hours are available.
  • A BA/BS in a mental health-related field is required; alternatively, a minimum of two years (4000 hours) of verifiable experience providing direct services to individuals with severe mental illness is acceptable.
  • Proficient computer skills in a Microsoft Windows environment, including email and internet navigation, are essential. Experience with Human Service Information Systems is highly desirable.
  • Ability to maintain confidentiality and professionalism while representing the organization positively.
  • Exhibit maturity, sound judgment, and emotional stability.
  • Strong problem-solving skills with the ability to analyze information effectively.
  • Excellent customer service skills, including managing challenging situations and responding promptly to client needs.

Physical and Environmental Requirements:

  • Physical: May occasionally require lifting/pulling objects up to 50 lbs and carrying items up to 25 lbs. Must be able to walk, stand, and sit for extended periods.
  • Sensory: Must be able to read documents and communicate effectively in English.
  • Cognitive: Ability to concentrate on detailed tasks with frequent interruptions.
  • Environmental: Exposure to various office and mixed environments, including potential exposure to toxins and loud noises.

Special Requirements:

  • Must pass a criminal records clearance as required by licensing regulations.
  • A valid California driver’s license and reliable personal vehicle are essential for this position.
  • Must pass pre-employment medical and drug tests as required.
  • Possession of a personal cell phone with reliable service is necessary for business communication.
  • First Aid and CPR certification may be required.
