Social Services Assistant

We are seeking a dedicated and compassionate Social Services Assistant to join our team at Oak Grove Center in Waterville, ME. As a Social Services Assistant, you will play a vital role in enhancing the lives of our patients/residents by identifying their strengths, social, emotional, and mental health needs, and connecting them with necessary services.
Key Responsibilities:- Collaborate with our interdisciplinary team to promote and protect patient/resident rights and well-being.
- Develop and implement written, interdisciplinary plans of care to address social service needs.
- Provide support and information to patients/residents, families, and significant others to improve quality of life.
- Perform discharge planning activities, including family/patient education, nursing center documentation, and referrals to community-based agencies and services.
- Establish and maintain relationships with community-based agencies and services that benefit our patient/resident population.
