About the Role

We are seeking a highly skilled and compassionate Social Services Director to join our team at Cumberland Village Center in La Follette, TN. As a key member of our interdisciplinary team, you will be responsible for fostering an environment that promotes the well-being and quality of life for our patients/residents.

Key Responsibilities
  • Develop and implement policies and routines that support patient/resident individuality, independence, and dignity.
  • Collaborate with social services staff, interdisciplinary team, and administration to promote and protect patient rights and dignity.
  • Conduct comprehensive psychosocial assessments to identify social, emotional, and psychological needs and strengths.
  • Educate patients and families regarding their rights and responsibilities, healthcare decision making, and available community resources.
  • Provide therapeutic interventions to assist patients with transition and adjustment to our nursing center.
Qualifications
  • Bachelor's degree in social work or human services required.
  • Must possess any certifications/licensures as required by state of employment to practice in long-term care.
