Community Health Worker

Westside Family Healthcare is seeking a dedicated Community Health Worker to join our Social Services team. As a core member of our collaborative care team, you will provide social service case management directly in the communities through outreach to specific target populations.

Key Responsibilities:
  • Identify social, emotional, environmental, financial, and other support needs for community members and established patients, and connect them with appropriate services and community resources.
  • Work closely with our multidisciplinary teams and community partners to identify areas of need and resources, with the goal of improving patients' health care and outcomes.
  • Coach community members and established patients in effective management of their chronic health conditions, self-care strategies, and motivate patients to be active, engaged participants in their health care.
  • Maintain accurate and timely documentation.
Requirements:
  • High School Diploma or General Equivalency Diploma (G.E.D.)
  • Fluency in written and spoken English and Spanish and/or Haitian Creole
  • Related work or volunteer experience providing services to a culturally diverse population
  • Verifiable good driving record and reliable transportation
  • Ability to travel independently between offices and communities throughout Delaware as needed
  • Ability to perform duties directly in the communities as needed (up to 90% of the schedule will be required to work in communities throughout Delaware)
  • Ability to work evenings and weekends as needed
  • Experience using computers and Microsoft Office software
