Job Summary

We are seeking a skilled Social Worker III to join our Rehabilitation Services team at AdventHealth Winter Park. As a key member of our team, you will play a vital role in coordinating social work and case management services to patients and their families, ensuring a seamless transition from admission to discharge.

Responsibilities
  • Evaluate and establish individualized discharge plans of care, taking into account the psychological, social, functional, and financial impact of illness on patients and families.
  • Act as a liaison to payer sources, obtaining authorization and updating as needed to promote efficient utilization of healthcare services.
  • Facilitate patient movement through the program by providing proactive planning and creating solutions to ensure seamless care.
  • Coordinate contact and refer patients to appropriate agencies, services, and resources to set up post-discharge needs, including durable medical equipment and follow-up care.
  • Document accurately, completely, and timely, using measurable and objective terminology, following standards of practice, insurance guidelines, and department/hospital policy and procedures.
Requirements
  • RN or Bachelor's degree in Rehabilitation or Social Services.
  • Experience in Rehabilitation preferred.
  • Current valid State of Florida or multi-state license as a Registered Nurse, if applicable.
  • Basic Life Support (BLS) certification.
