Full Time Gerontology Nurse Practitioner Position in Albany, NY

We are seeking a skilled Gerontology Nurse Practitioner to join our team for a local contract position in Albany, New York.
Job OverviewThis is a 26-week assignment, working 40 hours per week, with an 8-hour day shift. As a Gerontology Nurse Practitioner, you will be responsible for providing primary, preventive, and urgent care to residents on-site, managing both chronic and acute needs.
Responsibilities- Conduct thorough clinical assessments for each resident on a monthly basis, with increased frequency for high-risk patients.
- Collaborate with an RN to coordinate care, ensuring timely follow-ups, communication, and continuity of care across facilities.
- Prescribe or refill non-narcotic medications after consulting with the resident's primary care provider.
- Offer single visits to patients transitioning from the hospital to the SNF, assessing and preparing them for a safe and successful discharge.
We offer a competitive salary of $120,000 per year, as well as medical, dental, and vision benefits.
The estimated salary for this position is approximately $150 per hour, based on a 40-hour workweek.
