Position Summary

The Case Manager will play a vital role in recommending and facilitating access to various services including medical, educational, social, vocational, and housing support to achieve program objectives. This position involves initiating referrals for advanced care, collaborating with healthcare providers for comprehensive care, and advocating for clients' best interests.

Key Responsibilities
  • Deliver case management services that include crisis intervention, discharge planning, individual and group counseling, mental health and substance use disorder support, screening, and assisting clients in fulfilling their basic needs and managing symptoms.
  • Ensure timely and accurate documentation while meeting minimum billing requirements for direct client services.
  • Facilitate client placements, including pre-placement visits and necessary transportation.
  • Participate in on-call team coverage as required.
  • Provide field-based services tailored to program and client needs.
  • Uphold client confidentiality at all times.
  • Engage in scheduled program meetings and contribute to team discussions.
  • Perform other relevant duties as assigned.
Qualifications
  • A Master's degree or a Bachelor's degree with four years of experience in social services is required. Acceptable credentials include ASW, MSW, MFT, LMFT, LPCC, or APCC.
  • For Substance Use Disorder Case Management, certification from a recognized board is preferred, and registration is required.
  • A valid driver's license and current auto insurance are mandatory.
  • Experience in providing mental health and/or substance use disorder services is preferred.
  • Cultural competency is essential.
  • Bilingual candidates are preferred.
  • Familiarity with Microsoft Office and EHR software is advantageous.
