Job OverviewWe are seeking an experienced Care Manager to join our leadership team at Assisting Hands Serving Carlsbad and Oceanside. The successful candidate will play a pivotal role in overseeing the delivery of care solutions and services to our clients, working directly with our owners to coordinate care plans, assess client needs, supervise caregivers, and serve as a liaison between clients, caregivers, and other healthcare professionals.
Responsibilities- Conduct comprehensive assessments of client needs and develop individualized care plans in collaboration with clients, their families, and other healthcare professionals.
- Coordinate and schedule care services based on client preferences, availability of caregivers, and specific care requirements.
- Recruit, train, supervise, and evaluate caregivers to ensure they deliver exceptional care in accordance with company policies, procedures, and standards.
- Conduct regular home visits to assess client satisfaction, monitor the effectiveness of care plans, and address any concerns or issues that may arise.
- Serve as a point of contact for clients and their families, providing support, guidance, and advocacy as needed.
- Collaborate with healthcare providers, including physicians, nurses, therapists, and social workers, to coordinate and effectively integrate care services.
- Maintain accurate and up-to-date documentation of client assessments, care plans, and service delivery in accordance with regulatory requirements and company standards.
- Minimum of 3 years' experience as an LVN and/or experienced homecare manager.
- Skilled problem solver with a can-do attitude.
- Strong interpersonal, communication, and leadership skills with the ability to collaborate effectively with clients, caregivers, and office team members.
- Proficiency in computer skills, including Microsoft Office applications. Experience with care management and scheduling software preferred.
- Compassionate, empathetic, and client-centered approach to care management.
- Valid driver's license and reliable transportation.
$45.00 per hour
