Job Summary

Eaton County Health & Rehabilitation Services is seeking a skilled Licensed Practical Nurse to join our team. As a LPN, you will be responsible for providing high-quality patient care and supervision to our residents.

Key Responsibilities
  • Supervise and Direct Nursing Activities: Supervise and direct the day-to-day functions of Certified Nursing Assistants (CNAs) and resident care, in accordance with current policies, procedures, rules, regulations, and guidelines that govern long-term care facilities.
  • Participate in Admission, Transfer, and Discharge: Participate in the admission, transfer, and discharge of residents, complying with regulations, standards of care, and policies.
  • Complete and Follow Through on Physician's Orders: Complete and follow through on physician's orders per facility policy and in accordance with regulators in scope of practice.
  • Perform Routine Charting Duties: Perform routine charting duties as required and in accordance with regulations, standards of care, and facility policy and procedures.
  • Dispense Medications and Treatments: Follow the standards of practice when dispensing medications and treatments, along with Pharmacy, State, Federal, and facility guidelines, policies, and procedures.
  • Develop and Maintain a Good Working Rapport: Develop and maintain a good working rapport with neighborhood residents, family, and personnel, as well as others within and outside the facility to ensure that nursing services can be adequately maintained.
  • Rounds with Physicians: Rounds with physicians and notify of any changes to physicians, physician offices, as per Federal and State Regulations, Standard of Care, and facility policy.
  • Provide Nursing Care: Provide nursing care per scope of practice to residents that follow Federal and State Regulations and facility policy. This will be done timely, accurately, as prescribed by physicians or facility policy.
  • Meet with Residents and Family Members: Meet with residents and/or family members as necessary. Report problem areas to the Clinical Mentor and Social Mentor.
  • Recommend Equipment and Supply Needs: Recommend to the Clinical Mentor/Social Mentor the equipment and supply needs of your unit/shift.
  • Review Care Plans and Cardex: Review care plans and cardex daily to ensure that appropriate care is being rendered, that goals, problems, approaches, and revisions are based on the residents' needs.
  • Inform and/or Update Changes: Inform and/or update any changes that need to be made on the care plan to Clinical Mentor.
  • Assist in Revising MDS: Assist the Clinical and Social Mentor in revising the Minimum Data Set (MDS), including corresponding supplemental assessments for accurate MDS coding and implementation of Triggers and RAPs.
  • Maintain Resident Rights: Ensure residents' rights are maintained per facility policy, Federal and State Regulations.
  • Other Duties: Perform all other duties as assigned by the Clinical and/or Social Mentor.
Qualifications
  • Genuine Interest in Geriatric Work: Must have a genuine interest in geriatric work.
  • Ability to Cooperate with Others: Must have the ability to cooperate with others and use tact and patience in dealing with residents, their families, and visitors.
  • Flexibility and Adaptability: Ability to be flexible to meet changing demands as necessitated by variations in the nursing home routine. Capable of adapting to frequent changes in routine.
  • Competent Operation of Mechanical Lift: Able to competently operate a mechanical lift.
