Job Summary

Eaton County Health & Rehabilitation Services is seeking a skilled Licensed Practical Nurse to join our team. As a LPN, you will provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.

Key Responsibilities
  • Provide direct nursing care to residents, including administering medications, treatments, and performing routine charting duties.
  • Supervise and direct the day-to-day functions of Certified Nursing Assistants (CNAs) and resident care, ensuring compliance with current policies, procedures, rules, regulations, and guidelines.
  • Participate in admission, transfer, and discharge of residents, adhering to regulations, standards of care, and policies.
  • Complete and follow through on physician's orders, as per facility policy and in accordance with regulators' scope of practice.
  • Develop and maintain a good working rapport with residents, family members, and personnel to ensure that nursing services can be adequately maintained.
  • Rounds with physicians, notifying them of any changes, as per Federal and State Regulations, Standard of Care, and facility policy.
  • Meet with residents and/or family members as necessary, reporting problem areas to the Clinical Mentor and Social Mentor.
  • Recommend equipment and supply needs to the Clinical Mentor/Social Mentor.
  • Review care plans and cardex daily to ensure that appropriate care is being rendered, that goals, problems, approaches, and revisions are based on the residents' needs.
  • Inform and/or update any changes that need to be made on the care plan to the Clinical Mentor.
  • Assist the Clinical and Social Mentor in revising the Minimum Data Set (MDS), including corresponding supplemental assessments for accurate MDS coding and implementation of Triggers and RAPs.
  • Ensure residents' rights are maintained per facility policy, Federal and State Regulations.
Requirements

To be successful in this role, you must have a strong understanding of nursing practices, policies, and procedures. You should be able to work effectively in a team environment and communicate clearly with residents, family members, and personnel.
