Job Title: CAMH Case Manager

Job Summary:

We are seeking a highly motivated and compassionate CAMH Case Manager to join our team at Texoma Community Center. As a key member of our mental health services team, you will provide case management services and rehabilitation skills training to children and adolescents with emotional disturbance and/or mental illness.

Key Responsibilities:

  • Provide case management services, including advocacy, crisis services, individual and group skills training, and other tasks as assigned by program managers.
  • Operate under trauma-informed and recovery-oriented models of care, aligning with CCBHC standards.
  • Collaborate with individuals, families, and community stakeholders to promote access to intellectual, developmental, and behavioral health services.
  • Develop and implement individualized treatment plans, ensuring a comprehensive and person-centered approach.
  • Provide education and support to individuals and families, promoting self-determination and empowerment.

Requirements:

  • Bachelor's degree in Social Work, Sociology, Psychology, Education, Criminal Justice, or Counseling.
  • Qualify for QMHP-CS certification.
  • Two years' human services experience preferred.
  • Experience working with individuals with mental illness and/or addiction issues desirable.
