Job Summary:

We are looking for a dedicated and caring LPN to join our team of healthcare professionals. The successful candidate will be responsible for providing skilled nursing care to patients in their homes, under the supervision of a Registered Nurse. This is a rewarding opportunity to work in a dynamic and supportive environment, where you will have the chance to develop your skills and advance your career.

Responsibilities:
  • Provide one-to-one patient care in a home environment, adhering to the developed care plan and physician's orders.
  • Conduct patient assessments, coordinate care, and document all activities, assessments, and nursing actions in a timely manner.
  • Administer prescribed medication, treatments, and therapies, as directed by the physician.
  • Participate in the development and update of the nursing care plan, ensuring that patient needs are met and goals are achieved.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
Requirements:
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
