We are seeking a highly skilled RN Home Care Specialist to join our team at PeaceHealth. This is a per diem/relief position, 0.00 FTE, Day shift.

The estimated salary for this role is $65,055 per year.

As a Home Health Nurse, you will work closely with patients and their families to develop and implement personalized care plans that address their unique needs and goals.

Key Responsibilities:

  • Assess patient's condition and develop a comprehensive plan of care that addresses their medical, emotional, and social needs.
  • Collaborate with other healthcare professionals to ensure seamless transitions of care and optimal patient outcomes.
  • Provide skilled nursing care and education to patients and their caregivers, including wound care, medication management, and disease prevention.
  • Conduct regular evaluations and make adjustments to the care plan as needed to ensure optimal patient outcomes.
  • Maintain accurate and detailed records of patient interactions, progress, and outcomes.

Requirements:

  • Graduate of an accredited school of nursing required. BSN preferred.
  • Two years acute clinical or home health/hospice nursing experience required.
  • Current Registered Nurse licensure in the state in which nurse will be practicing.
  • Current BLS (Basic Life Support) certification required at date of hire.
  • Valid driver's license (OR, WA or AK as applicable) and proof of auto insurance required.
