Key Responsibilities- Provide education to patients discharging from area hospitals, focusing on infusion therapy and related care.
- Deliver infusion nursing care in the home or clinic, collaborating with various providers, patients, and caregivers to ensure seamless care.
- Develop and implement individualized care plans, taking into account patient needs, medical history, and treatment goals.
- Conduct thorough assessments, identify potential risks, and implement strategies to mitigate them.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care outcomes.
- Current Oregon Registered Nurse License or Washington Registered Nurse License (Vendor Managed) or National Registered Nurse Compact License.
- National Provider BLS - American Heart Association certification within 30 days of hire.
- Unencumbered Registered Nurse License in state(s) of hire and wherever care is delivered.
- 1 year of experience in acute hospital care, infusion, or home health.
