Nurse Practitioner/Physician Assistant

The Resource Center is seeking a highly skilled Nurse Practitioner/Physician Assistant to join our team in the Community Inclusion department. As a key member of our primary care team, you will be responsible for providing high-quality medical care to patients in our clinical sites at TRC Community Health Center and/or School Based Health Centers in the Jamestown City School District.

Key Responsibilities:

  • Provide comprehensive patient care, including health promotion and disease prevention, management of acute and chronic diseases, and coordination of care between primary care and other medical services.
  • Develop and implement individualized plans of care, including the prescription of pharmacologic agents and non-pharmacologic therapies.
  • Record patient medical histories and maintain accurate and up-to-date medical records.
  • Collaborate with primary care physicians and other healthcare professionals to ensure seamless care coordination.
  • Participate in performance improvement initiatives and contribute to the development of quality improvement projects.
  • Provide education and support to patients and their families on health-related issues and promote healthy behaviors.

Requirements:

  • Current licensure as a Nurse Practitioner or Physician Assistant in the state of New York.
  • Experience working in a primary care setting, preferably in a community health center or school-based health setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work autonomously and collaboratively, with a strong commitment to teamwork and patient-centered care.
  • Knowledge of primary care protocols and procedures, including New York State Health Department regulatory standards.
