Job Title: Hospice RN Case Manager

We are seeking a dedicated and skilled full-time RN Case Manager to join our West team, serving counties West of Charlottesville, Virginia.

Job Summary

The Hospice RN Case Manager will play a vital role in ensuring the comfort, dignity, and well-being of our patients. This position requires a compassionate and empathetic individual with excellent clinical assessment and critical thinking skills.

Responsibilities
  • Perform nursing evaluation visits, develop plans of care in conjunction with physician's orders, and coordinate with interdisciplinary team members to ensure optimal comfort and quality of life.
  • Assess and re-evaluate patient needs, consulting with physicians, reporting changes, and communicating with others when a change in the Plan of Care is necessitated.
  • Provide education to patients, families, and caregivers about disease progression, symptom management, and end-of-life care options.
  • Coordinate care with other healthcare professionals, including physicians, social workers, chaplains, and CNAs, to ensure a solid team approach to patient care.
  • Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions using the Electronic Medical Record (EMR).
  • Participate in interdisciplinary team meetings, providing valuable input and contributing to the overall care plan.
  • Stay current with best practices, research, and developments in hospice care, seeking continuous professional development opportunities.
Requirements
  • Current and unrestricted Registered Nurse (RN) license in the state of Virginia.
  • Minimum of 2 years of nursing experience preferred, preferably in hospice, palliative care, or a related field.
  • Knowledge of hospice regulations, practices, and standards of care with a demonstrated ability to manage all aspects of terminal care, health team service coordination, documentation, and case management.
  • Excellent clinical assessment and critical thinking skills, emotionally mature with the ability to cope with family stressors and tolerance of individual lifestyles.
  • Strong communication and interpersonal skills, with the ability to establish rapport with patients, families, and healthcare professionals.
  • Compassionate and empathetic nature, with a genuine desire to provide comfort and support during challenging times.
  • Ability to work independently and as part of a team, collaborating effectively with interdisciplinary professionals.
  • Highly organized, self-motivated with the ability to articulate information with a diverse population, and proficient in using electronic medical records (EMR) and other healthcare software.
