We are seeking a highly skilled Registered Nurse (RN) Case Manager to join our team at Enhabit Home Health & Hospice. As a RN Case Manager, you will be responsible for providing high-quality patient care and coordinating with healthcare professionals to ensure seamless care delivery.
Key Responsibilities- Perform home visits and coordinate client care to ensure optimal patient outcomes.
- Oversee the frequency of visits for each episode and consult with physicians as needed.
- Collect accurate OASIS data, maintain current medication profiles, and report lab values to physicians in a timely manner.
- Attend weekly case conferences and monthly case manager meetings to stay informed and share best practices.
- Current licensure as a Registered Nurse (RN) in the state of employment.
- A minimum of one year of clinical experience preferred.
- Demonstrate knowledge and skill in current nursing practice.
- Valid state driver's license and automobile liability insurance.
