About the Role:

This position involves facilitating the development, implementation, coordination, and evaluation of individualized patient plans triggered by referrals from nursing, physician, interdisciplinary rounds, and other care team members.

You will collaborate closely with patients, families, physicians, and the interdisciplinary team to ensure high-quality care is provided across different levels of care. As a key member of the care team, you will take a leadership role in achieving actions and processes designed to ensure optimal clinical outcomes, coordination, and continuity of health care.

The ideal candidate will possess strong communication skills, be able to interact effectively with diverse populations, and demonstrate a deep understanding of disease processes, treatment protocols, and pharmacological management.

Requirements:
  • Education: Graduate of an accredited BSN school of professional nursing or a school accepted for candidacy status from an accrediting body.
  • Licensure: Registered Nurse, Licensed in the State of Ohio.
  • Experience: Two (2) years recent experience in clinical, utilization management, home care, discharge planning, managed care, and/or case management that has provided a broad understanding in Case Management and Utilization Management concepts.
