Job Summary:
The Complex Care Social Worker will work closely with patients, families, and healthcare teams to identify and address barriers to discharge. Using a systematic approach, you will gather pertinent information, develop and implement effective care plans, and collaborate with the team to ensure timely and cost-effective discharges.
Key Responsibilities:
- Conduct thorough assessments of patients' needs and develop individualized care plans.
- Collaborate with the interdisciplinary team to ensure seamless care coordination.
- Identify and address barriers to discharge, including social determinants of health.
- Provide education and support to patients and families to promote self-advocacy and empowerment.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed.
Requirements:
- Master's degree in Social Work or related field.
- Current licensure as a LCSW-C or LCPC in the State of Maryland.
- Minimum of 3 years of experience in social work or a related field.
