Key Responsibilities:

  • Conduct thorough assessments of patients' needs and develop individualized care plans.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination.
  • Identify and address barriers to discharge, including social determinants of health.
  • Provide education and support to patients and families to promote self-advocacy and empowerment.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed.

Requirements:

  • Master's degree in Social Work or related field.
  • Current licensure as a LCSW-C or LCPC in the State of Maryland.
  • Minimum of 3 years of experience in social work or a related field.
