We are seeking a highly skilled and compassionate Care Coordinator RN to join our team at Carle Health. As a key member of our population health management team, you will play a vital role in providing care management and coordination services to patients within our assigned region.
Key Responsibilities- Care Planning and Coordination: Conduct in-depth assessments of patient/family needs, coordinate input from all health professionals, and formulate a documented plan to ensure continuity of care for at-risk patient populations.
- Delegation and Accountability: Delegate care based on the situation while assuming accountability for patient outcomes, support assistive personnel, and serve as a resource to hold care teams accountable for delegated tasks.
- Advance Care Planning: Connect patients and surrogate decision-makers to the advance care planning facilitation process, ensuring that advance care planning documents are stored and available within the electronic health record.
- Medication Management: Reconcile discharge medication orders, collaborate with PCPs and interdisciplinary team members on medication changes, and ensure patient understanding of medications to stop or initiate.
- Psycho-Social Support: Identify complex behavioral or social needs, make appropriate referrals, and collaborate with physicians and interdisciplinary teams to evaluate progress, identify barriers, and opportunities to improve care.
- Transition Coordination: Coordinate and manage transitions of care across the continuum to ensure appropriate utilization of clinical and community resources.
- Data Analytics: Identify risk stratification via EHR encounters or datasets to intervene as appropriate, integrate patient registry, stratification, and other tools/reports to identify patients who may be appropriate for care management.
- College diploma in Nursing
- Registered Nurse (RN) licensure in Illinois and Basic Life Support (BLS) certification
- Three (3) years of clinical experience as a registered nurse, with relevant clinical experience and current knowledge of healthcare trends related to emerging care models and population health management
