Key Responsibilities:
- Oversee patient admissions as a Resource Care Coordinator (RCC).
- Act as a resource for other RCCs during the admissions process.
- Facilitate communication and coordination of care among all team members.
- Advocate for patients and assist care team members throughout the care continuum.
- Support the market manager in staffing coordination and resource allocation for patient care.
- Ensure new RCCs have the necessary tools and templates for their roles.
- Assist in documenting clinical and operational workflows.
- Help maintain data accuracy and reporting standards.
- Identify and address negative trends in the healthcare system, proposing solutions to challenges.
- Foster collaborative relationships with all care team members, providing guidance during critical situations.
- Participate in the recruitment, interviewing, and training of new staff as needed.
- Perform additional duties as assigned.
Qualifications:
Required:
- Associate Degree in Nursing.
- Current and active RN state licensure.
- Three to five years of experience in acute nursing, with a solid understanding of clinical standards and outcome measurement.
- Reliable transportation and proof of liability insurance.
Preferred:
- Bachelor of Science in Nursing (BSN) preferred.
- Certification in Care Coordination and Transition Management (C.C.C.T.M).
- Familiarity with the geographical area of the assigned market.
- Experience as an acute care RN providing in-home care for at least three months.
- Two to three years of experience as a recovery care coordinator.
