Key Responsibilities:

  • Oversee patient admissions as a Resource Care Coordinator (RCC).
  • Act as a resource for other RCCs during the admissions process.
  • Facilitate communication and coordination of care among all team members.
  • Advocate for patients and assist care team members throughout the care continuum.
  • Support the market manager in staffing coordination and resource allocation for patient care.
  • Ensure new RCCs have the necessary tools and templates for their roles.
  • Assist in documenting clinical and operational workflows.
  • Help maintain data accuracy and reporting standards.
  • Identify and address negative trends in the healthcare system, proposing solutions to challenges.
  • Foster collaborative relationships with all care team members, providing guidance during critical situations.
  • Participate in the recruitment, interviewing, and training of new staff as needed.
  • Perform additional duties as assigned.

Qualifications:

Required:

  • Associate Degree in Nursing.
  • Current and active RN state licensure.
  • Three to five years of experience in acute nursing, with a solid understanding of clinical standards and outcome measurement.
  • Reliable transportation and proof of liability insurance.

Preferred:

  • Bachelor of Science in Nursing (BSN) preferred.
  • Certification in Care Coordination and Transition Management (C.C.C.T.M).
  • Familiarity with the geographical area of the assigned market.
  • Experience as an acute care RN providing in-home care for at least three months.
  • Two to three years of experience as a recovery care coordinator.
