About the Position

As a Nurse Practitioner with Matrix Medical Network, you will play a critical role in improving the quality of care for at-risk individuals. You will conduct comprehensive health assessments in the comfort of patients' homes, reviewing their current health, medical history, medication adherence, social environment, and other risks. This provides unparalleled insight into patients' overall health and well-being, allowing us to identify potential care gaps and provide targeted interventions.

Responsibilities
  • Conduct home visits to assess patients' health and identify potential care gaps
  • Review patients' medical history, medication adherence, and social environment
  • Develop and implement personalized care plans to address identified needs
  • Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and self-management
Requirements
  • Masters degree in Nursing or related field
  • Current RN and NP licensure in state of practice, including prescription authority
  • Board certification in a Matrix-approved specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills
