Job Title: Case Manager II

Union County Community Action, Inc. is seeking a highly motivated and experienced Case Manager II to join our team. The successful candidate will be responsible for providing health, nutrition, and social services to families enrolled in our Head Start program.

Key Responsibilities:
  • Recruit and enroll families in our program
  • Complete all necessary paperwork and initiate Family Partnership Agreements
  • Follow up with families to ensure goals are met and services are provided
  • Assist with health screenings and provide transportation as needed
  • Make home visits to provide support services
  • Enter data into Child Plus and run weekly and bi-weekly reports
  • Follow up on health referrals and ensure completion of services
  • Complete CACFP quarterly food reviews
  • Attend parent meetings and activities
  • Assist with planning and presenting educational programs for families
  • Encourage parents to volunteer and keep records of volunteer time
  • Refer parents to appropriate agencies for needed services
  • Translate written correspondence between the agency and Spanish-speaking clients
  • Work collaboratively with site manager, family and community coordinator, and specialists to ensure coordinated delivery of services

Requirements:

  • Bachelor's degree in Human Services, Social Services, or a related field
  • 1-3 years of related experience and/or training
  • Ability to read, write, and speak English and Spanish
  • Ability to work collaboratively with families and staff

Working Conditions:

  • Work is performed in a fast-paced office environment
  • May require occasional travel to off-site locations
