Job Summary
We are seeking a skilled Case Manager - Youth Services Coordinator to join our team. This role will provide case management services and mental health rehabilitative services to severely emotionally disturbed clients and their families.
Key Responsibilities
- Provide case management services, including assessment, service planning, and review, to ensure the clinical team members have up-to-date information on client cases.
- Collaborate with teachers, school guidance counselors, and other service practitioners to ensure appropriate services for clients.
- Counsel clients as they progress through their levels of service need.
- Develop crisis backup support plans to ensure client safety and well-being.
- Promote parent and key player involvement in planning services and supports for each client.
- Develop strategies to overcome obstacles and challenges.
- Manage resource allocations in accordance with agency and school district regulations.
- Facilitate active communication among client service teams.
- Complete client medical record documentation and required forms within 24-72 hours.
- Participate in treatment planning and implementation through case management and mental health rehabilitation services.
- Comply with federal, state, accreditation, and agency policy requirements regarding case management services.
- Transport clients to/from home, school, the agency, and treatment meetings as needed.
- Encourage family participation in service planning and delivery.
- Assist clients and families in accessing support within the home, school, and community.
- Conduct foster home visits and biological home visits as outlined in the treatment plan.
- Maintain client confidentiality according to agency policy and federal law.
- Serve as a facilitator for client person-centered planning and Individualized Transition Plan (ITP) teams.
- Arrange necessary services and supports for clients according to their ITP.
- Interface with adult mental health service coordinators for continuity of age-appropriate services.
- Assist clients in acquiring access to community settings and resources.
Requirements
To be successful in this role, you will need:
- A Bachelor's Degree from an accredited college or university with one year of experience providing care coordination, collaborating with community service providers, and direct services to children and families.
- Excellent written and oral communication skills, sound clinical and therapeutic skills, and the ability to accurately document events and work with a multi-disciplinary team.
- Ability to maintain positive professional relationships and work effectively within a team environment.
- Organized, efficient, and able to manage multiple tasks simultaneously with attention to detail and accuracy.
- Ability to ensure client and employee confidentiality according to agency policy and federal law.
- Proficient with Microsoft applications and the ability to learn other software applications.
- Knowledge and understanding of agency protocol, policy, procedure, plans, program requirements.
- Ability to act decisively and with fairness.
- Ability to read, speak, and write in English.
Working Conditions
This role will involve working in a community mental health center office setting, schools, and client homes. You will need to maintain a valid Alaska Driver's License, current vehicle insurance, and registration on personal vehicles used for company business, as well as a violation-free driving record for the past year.
Estimated Salary: $50,000 - $65,000 per year
