About the Role:

We are seeking an experienced Patient Care Manager to lead our clinical team at Community Healthcare of Texas. As a Patient Care Manager, you will oversee the admission process, manage direct patient care, and ensure compliance with regulatory and accrediting agencies.

Key Responsibilities:

  • Oversight of patient admissions and conferences with team physicians
  • Management of patient care, including assignment of staff and scheduling
  • Review of patient medical diagnoses, prognoses, medications, procedures, and clinical courses
  • Assistance to clinicians in establishing therapeutic goals and developing plans of care
  • Attendance at case conferences and facilitation of coordination of care
  • Screening and interviewing of new employees and making recommendations for hire
  • Audit of patient charts and response to compliance issues
  • Maintenance of regulatory compliance and participation in quality assessment performance improvement teams

Required Skills and Qualifications:

  • College degree; Registered Nurse/BSN preferred; LMSW or LVN with applicable experience considered
  • 2+ years of hospice experience and supervisory experience required
  • Charge Nurse experience may meet supervisory requirements
  • Hospice regulations knowledge strongly preferred
  • Proficient computer skills, Microsoft Office suite experience required
  • Good communication and travel skills

Salary Estimate:$90,000 - $110,000 per year, depending on experience.
