Position Type: Per Diem - A minimum of four shifts within a six-week timeframe is required, with opportunities for additional shifts available.

Overview:

As a Licensed Practical Nurse (LPN) at Mary Free Bed Rehabilitation Hospital, you will play a vital role in delivering quality patient care within the Nursing Department. Under the supervision of the assigned registered nurse (RN), you will assist in patient care and contribute to departmental activities as outlined in this job description, ensuring effective communication with the responsible RN. Your work will align with the mission, vision, and values of our organization.

Key Responsibilities:

Quality of Care:

The LPN is responsible for providing direct patient care as delegated by the RN, adhering to departmental policies and procedures. Responsibilities include, but are not limited to:

  • Monitoring and reporting on the physiological, psychological, socio-cultural, and spiritual needs of patients and their families, in collaboration with the RN.
  • Administering medications (excluding IV administration) as prescribed by the physician and delegated by the RN.
  • Contributing to the development of individualized care plans for patients.
  • Reinforcing educational information for patients and families, documenting accordingly.
  • Measuring and recording vital signs accurately.
  • Providing comprehensive patient care, including hygiene, bowel programs, and other responsibilities as per departmental guidelines.
  • Responding promptly and respectfully to patient call lights.
  • Documenting care provided in compliance with departmental policies.
  • Safely transporting patients for diagnostic tests and therapy sessions as required.
  • Maintaining a clean and safe environment for patients.
  • Cleaning patient and work areas, reporting any equipment issues as necessary.
  • Assisting in the collection of quality data as directed by the RN or management.
  • Utilizing patient care equipment properly, following departmental protocols.
Communication: - All nursing staff must communicate in a timely, professional, and respectful manner. Responsibilities include:
  • Ensuring patients and their families are treated with kindness and respect.
  • Fostering positive relationships with customers, physicians, and colleagues.
  • Maintaining effective working relationships across the organization.
  • Responding promptly to patient call lights, treating them as a primary communication method.
  • Delivering culturally sensitive care while safeguarding patient dignity and confidentiality.
Customer Service: -
  • Demonstrating exemplary customer service in all interactions with patients, families, and colleagues.
  • Understanding patient satisfaction metrics and actively participating in improvement initiatives.
Employee Development: -
  • Engaging in ongoing communication with hospital and departmental leadership regarding policy and practice updates.
  • Completing required training and competency assessments annually.
  • Participating in performance evaluations and setting personal goals for professional growth.
Safety and Compliance: -
  • Adhering to safety and infection control policies to maintain a safe working environment.
  • Documenting patient chargeable items accurately and avoiding waste.
Qualifications: -
  • Active Michigan LPN license.
  • Graduation from an accredited practical nursing program.
  • Current Basic Life Support (BLS) certification.
  • Prior experience in a rehabilitation setting is preferred but not required.
Physical Requirements: -
  • Ability to stand, sit, talk, type, read, and hear.
  • Capable of handling, feeling, carrying, and working with hands and arms.
  • Frequent standing, walking, bending, kneeling, and rising from kneeling positions.
  • Independently mobile with the ability to lift and move objects as required.
  • Competency in using computer systems for patient documentation.
