Job Summary

We are seeking a skilled RN Care Coordinator to join our team at Brigham and Women's Hospital. As a key member of our care coordination team, you will play a vital role in ensuring the delivery of high-quality, patient-centered care.

Key Responsibilities
  • Coordinate and implement patient care plans, utilizing case management principles and evidence-based guidelines.
  • Collaborate with interdisciplinary teams to ensure seamless transitions of care and optimal patient outcomes.
  • Assess patient needs and develop individualized plans to address social determinants of health and promote health equity.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and shared understanding of care plans.
  • Monitor and evaluate patient progress, making adjustments to care plans as needed to ensure optimal outcomes.
  • Participate in quality improvement initiatives to enhance patient care and reduce healthcare disparities.
Requirements
  • Graduate of an approved school of nursing with current registration in Massachusetts.
  • Bachelor of Science Degree in Nursing required for newly licensed nurses and external candidates.
  • 1+ years of acute care experience and 1 year of case management experience required.
  • Strong clinical assessment skills, excellent interpersonal skills, and ability to work collaboratively within a team.
  • Ability to negotiate multiple aspects of care coordination simultaneously and demonstrate the ability to meet a patient's needs based on their clinical diagnosis, level of care, and discharge plan.
