Key Responsibilities- Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team
- Coordinate patient care services, including medical, emotional, and spiritual support
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
- Participate in ongoing comprehensive assessments, case conferences, and other forums to coordinate patient care
- Supervise and direct other team members in the planning and implementation of patient care
- Current licensure as a Registered Nurse in Massachusetts
- Associate's degree in nursing required
- Demonstrated ability to assess and respond to patient needs in varied settings
- Self-directed, flexible, and cooperative with a strong ability to work effectively within an interdisciplinary team
