Job Summary:

The CST Para Professional is a vital member of our Community Support team, working closely with consumers, family members, and community partners to provide personalized support and care. As a CST Para Professional, you will be responsible for interacting effectively with consumers, promoting relationships with family and friends, and supporting community-based opportunities.

Key Responsibilities:
  • Provide personalized support and care to consumers in their homes and communities.
  • Develop and maintain positive relationships with consumers, family members, and community partners.
  • Support consumers in attending professional appointments, life skills training, and rehabilitation programs.
  • Assist consumers in accessing community services and resources.
  • Participate in the development and implementation of individualized treatment plans.
  • Communicate effectively with team members and stakeholders to ensure seamless care coordination.
  • Maintain accurate records and charting, reporting unusual incidents in a timely manner.
Requirements:
  • Minimum one year experience supporting adult individuals with behavioral or mental health challenges and/or substance abuse issues.
  • High school diploma, GED, or equivalent.
  • Ability to write clearly and legibly.
  • Schedule flexibility to meet the needs of beneficiaries.
  • Knowledge of Behavior Management techniques, Substance Abuse counseling/support, and Recreation Leadership.
Work Environment:

The CST Para Professional will work primarily in the client's home and in the community, with occasional exposure to cigarette smoke, domestic animals/pets, and other environmental factors.

Physical Demands:

The CST Para Professional will be required to stand, walk, use hands to handle objects, talk and hear, and taste and smell. The employee must be able to lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds.
